Medication Management

Description:
Medication Management helps clients follow their prescribed medication routines safely and consistently. Many clients take multiple medications, have changing prescriptions, or may have difficulty remembering when and how to take their medication.

Our team can provide medication reminders, observe for concerns, help organize medication routines, communicate changes or concerns to family members, and support clients in understanding the importance of medication compliance. When nursing services are involved, a licensed nurse may provide additional medication monitoring and education according to the approved care plan.
